As a member of our team, you'll have the opportunity to make a positive impact on the lives of patients while enjoying competitive pay, comprehensive benefits, and the support of a dedicated clinical team. /n/nRequired Qualifications:/n/n /n/t - One year of recent experience as a Registered Nurse, including a minimum of six months in a Home Health setting as an RN/n/t - Proficient in OASIS documentation/n/t - Valid RN license in compliance with state regulations/n/t - BLS certification (AHA/ARC)/n /n/nPreferred Qualifications:/n/n /n/t - Home Health Nursing Certification (RN-BC)/n/t - Other certifications and licenses may be required for this position/n /n/nSummary:/n/n A Home Health Registered Nurse (RN) delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of their medical conditions. Home Health RNs assess patient conditions, develop care plans, administer treatments, and educate patients and caregivers on disease management. /n/nEssential Work Functions:/n/n /n/t - Assess patient conditions and develop individualized care plans based on medical needs and physician directives/n/t - Administer medications, treatments, and wound care while monitoring patient responses/n/t - Provide patient and caregiver education on medication management, disease progression, and self-care techniques/n/t - Monitor for changes in patient status, reporting concerns and adjusting care plans as needed/n/t - Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions/n/t - Document patient care, interventions, and progress accurately and in compliance with regulatory standards/n/t - Ensure compliance with infection control, safety, and home care best practices to protect patients and caregivers/n/t - Support families and caregivers, offering guidance and emotional support throughout the care process/n/t - Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care/n/t - Strong ability to complete charting and documentation requirements while meeting visit and productivity goals/n/t - Adhere to infection control, safety, and healthcare regulations/n/t - Perform other duties as assigned within the RN scope of practice and home health regulations/n /n/nRequired Essential Skills:/n/n /n/t - Critical thinking, service excellence and good interpersonal communication skills/n/t - The ability to read, write, & communicate in the English language is required/n/t - Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills/n/t - Physical Abilities - Remain in a stationary position, move about, move equipment (50-100lbs), pushing, pulling, bending/n/t - Interpersonal Skills - To work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism/n/t - Technical/Motor Skills - Ability to grasp, fine manipulation, pushing/pulling and able to move about when assisting with procedures and/or using department equipment./n/t - Mental Requirements - Must be able to cope with frequent contact with the general public, customers, and meeting deadlines under pressure. Must be able to work under close supervision occasionally as well as working without assistance from other personnel.
