difference. We are looking for a dedicated Nurse Navigator / LVAD
Coordinator like you to be a part of our team. Job Summary and
Qualifications Full time (Monday - Friday, day shift), LVAD call
rotation by schedule The Nurse Navigator / LVAD Coordinator is a
Registered Nurse RN who possesses advanced knowledge and skill in
cardiac nursing. Makes decisions of moderate difficulty. Handles
confidential information regarding patient records. Operates and
analyzes telemetry cardiac monitors, exercise equipment and patient
evaluation tools. May be required to make critical decisions
concerning the initiative of emergency medical procedures. We want
you to apply now. What you will do in this role: Assists in the
daily operations of the outpatient heart failure clinic. Documents
patient treatment regularly and accurately according to
departmental policy. Complete clinical evaluation of potential
candidates for ventricular assist device intervention. Initiates
advanced cardiac life support measures according to SAMC protocols
in the event of a cardiac emergency. Participate in weekly
selection committee meetings and monthly steering committee
meetings Provides appropriate education to all patients.
Demonstrates effective communication and organizational skills, and
evidence of professional development. Familiar with current
standards of practice and clinical and inpatient protocols.
Performs miscellaneous job duties as assigned in the absence of the
nurse manager or supervisor. Maintains a flexible working schedule
to allow for clinic coverage. Provide preoperative patient and
family education for device candidates. Collaborate with Patient
Financial Representatives about insurance coverage for the device
prior to surgery. Collect data required by the hospital, the FDA,
and other regulatory bodies related to implanted devices and submit
to the appropriate agency(ies). Provide post-operative education to
patient and family in collaboration with nursing staff in
preparation for discharge from the hospital (when possible based on
device implanted). Provide support and education to nursing staff
and other health professionals regarding the device, its
implications for the patient, and how to troubleshoot alerts and
alarms. Provide clinical support to patients leaving the unit for
tests and procedures. Verify patient knowledge base prior to
discharge from hospital. Serve as resource person to patients after
discharge for clinical and technical support. Provide support and
education to community health care professionals who will have
contact with the patient. Interview and treat patients during
clinic appointments within scope of practice. Assist in development
and integration of new and existing practices into the VAD Program
Policy and Procedure Manual. Assist with maintenance of adequate
supplies for patients on device and for potential implants.
Actively participate in local and national meetings to exchange
knowledge and information in the area of cardiac surgery including
cardiac assist devices. Perform other related duties as assigned.
What Qualifications you will need: EDUCATION: Graduate of an
approved school for registered nurses and current state required
license to practice nursing. Advanced cardiac life support (ACLS)
certification preferred. Certification as a heart failure nurse
preferred (CHFN) EXPERIENCE: Minimal three years' experience as a
RN with 1-2 years' experience working with cardiac patients,
preferably on a telemetry unit or coronary care unit required. Must
have been directly involved in education and care of patients with
coronary heart disease, including heart failure. Previous LVAD
experience preferred. CERTIFICATION/LICENSE: Current Registered
Nurse license to practice nursing in the State of employment BLS
