How you’ll make an impact in this role - Lead the implementation and continuous monitoring of personalized care plans, ensuring every patient receives a high standard of treatment tailored to their specific recovery goals. - Coordinate care across multiple clinical disciplines, acting as the central hub for communication to ensure the entire healthcare team is aligned and working toward the best patient outcome. - Assess and facilitate the patient’s journey from admission to discharge, collaborating with the healthcare team to ensure a safe, seamless, and well-prepared transition back to the community. - Communicate critical changes in patient condition to the medical team in real-time, providing the precise clinical insights necessary for rapid and accurate intervention. What minimum requirements you’ll need Licensure / Certification / Registration: - Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Pediatric Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required. Education: - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
